Social Prescribing is an innovative approach that helps older adults improve their health and well-being by connecting them with local activities, programs, services, and community supports. Recognizing that health is influenced by more than medical care alone, Social Prescribing addresses social determinants of health such as loneliness, isolation, community involvement, and access to support networks.
Rather than simply providing medication or treatment, healthcare and social care professionals can refer individuals to a Community Connector who works one-on-one with participants to identify their interests, goals, and needs, creating a personalized wellness plan that helps them stay active, engaged, and connected.
What is a Community Connector?
A Community Connector meets with older adults to:
- Learn about their interests, goals, and challenges
- Develop a personalized wellness plan
- Connect them with relevant local programs, services, and activities
- Support participation in community opportunities such as volunteering, recreation, social groups, and educational programs
- Provide referrals and ongoing support as needed
Who Can Participate?
The Social Prescribing program is designed for adults 55 years of age and older who may benefit from additional social connections, community involvement, or support services.
Referrals can be made by:
- Physicians and nurse practitioners
- Nurses and social workers
- Other healthcare professionals
- Community organizations
- Family members
- Self-referrals (where available)
How the Social Prescribing Pathway Works
- Older Adult Identified
An older adult experiencing social, emotional, or wellness challenges is identified. - Referral Made
A healthcare provider, social service provider, family member, or community organization refers the individual. - Community Connector Consultation
The Community Connector meets with the participant to understand their needs and interests. - Personalized Wellness Plan
A customized plan is created and appropriate programs, activities, and services are recommended. - Connection to Community Resources
Participants are supported in accessing local opportunities and services. - Follow-Up and Evaluation
The Community Connector follows up to ensure needs are being met and shares updates with referral partners when appropriate.
